Abstract: In der Studie wurde die Auswirkungen von Trauma auf die Schwere der Symptome bei paranoider Schizophrenie und das Auftreten psychose-ähnlicher Symptome bei emotional-instabiler Persönlichkeitsstörung betrachtet. Es wurden zwei Patientengruppen mit den Diagnosen paranoide Schizophrenie (n=31) bzw. emotional-instabile Persönlichkeitsstörung (n=23) sowie eine gesunde Kontrollgruppe (KG; n=27) untersucht. Dazu wurden traumatische Erlebnisse und Positivsymptome erfasst. Traumata traten in den Patientenstichproben signifikant häufiger auf als in der KG. Sie erklärten in der Patientenstichprobe 20% der Varianz in der Positivsymptomatik. Die Qualität des Traumas korrelierte bei gewalttätigen Angriffe durch Bekannte und Familie (Zeuge), sexueller Missbrauch in Kindheit und Jugend (persönlich) und Folter (persönlich) signifikant in einer mittleren Effektstärke. Die Wahrscheinlichkeit für das Auftreten von Positivsymptomatik steigt unter Auftreten von traumatischen Ereignissen. Implikationen für eine traumatherapeutische Behandlung können diskutiert und in weiteren Studien verifiziert werden.
The aim of this study was to investigate the impact of trauma on symptom severity in patients with paranoid schizophrenia and the occurrence of psychotic-like symptoms in patients with emotionally unstable personality disorder. Two groups of patients diagnosed with paranoid schizophrenia (n = 31) or borderline personality disorder (n = 23) and a healthy control group (n = 27) were examined. For this purpose traumatic experiences and positive symptoms were recorded. Traumatic experiences were significantly more frequent in the patient samples compared to the control group. Those experiences explained 20% of variance in connection with positive symptoms in the patient sample. The quality of trauma correlated significantly in the domain of violent attacks by acquaintances and family (witness), sexual abuse in childhood and youth (personal) and torture (personal) in a medium effect size. The probability of the onset of positive symptoms increases with the occurrence of traumatic events. Implications for trauma therapeutic treatment methods can be discussed and verified in further studies.
