Abstract: Die chronisch progressive externe Ophthalmoplegie (CPEO)entsteht durch singuläre Deletion der mitochondrialen DNA(mtDNA) oder durch Mutation im nukleären Gen, welche mit multiplen Deletionen der mtDNA einhergeht. Eine besondere Form der CPEO mit Multiorganbeteiligung stellt das Kearns-Sayre-Syndrom (KSS) dar. In der Untersuchung der singulären mtDNA-Deletionen bei 25 Patientenmit CPEO zeigte sich keine Korrelation zwischen Phänotyp und molekulargenetischen Befunden wie Gewebedistribution oder Länge der Deletion sowie Heteroplasmiegrad im Muskel. Ein früherer Krankheitsbeginn korrelierte signifikant mit Deletionnachweis in einem mitotischen Gewebe (Blut oder Urothelzellen) und nicht signifikant mit höherem Heteroplasmiegrad im Muskel. Die multiple Deletionen waren in Untersuchung der mtDNA von 15 Patienten nur im Muskel nachweisbar. Bei einem Patient mit MELAS-Phänotyp und multiplen Deletionen der mtDNA im Muskel konnten zwei heterozygote POLG1-Punktmutationen identifiziert werden. Die Sensitivität der Long-Range-PCR-Untersuchung konnte durchVerkürzung der Elongationszeit erhöht werden.
The chronic progressive external ophthalmoplegia (CPEO) is caused by single deletion of mitochondrial DNA (mtDNA) or by mutation within the nuclear gene which is accompanied by multiple deletions of the mtDNA. A special form of CPEO with multi-organ-involvement is the Kearns-Sayre syndrome (KSS). The evaluation of the single mtDNA-Deletions in 25 patients with CPEO showed no correlation between phenotype and moleculargenetic findings like tissue distribution or deletion length or the degree of heteroplasmy in muscle. An early onset of the disease correlated significantly with detection of deletion in one of mitotic tissues (blood or urothel epithelial cells) but not significantly with a higher degree of heteroplasmy within the muscle. In the examination of the mtDNA of 15 patients multiple deletions could only be proven within muscle tissue. In one patient with MELAS-phenotype multiple deletions of the mtDNA in the muscle were identified caused by two heterozygous POLG1-point-mutations. The sensitivity of the long-range-PCR-analysis was increased by shortening the elongation time.
