Abstract: Die intraoperative Radiotherapie (IORT) mit dem Intrabeam®-System (Carl Zeiss AG) stellt eine neue therapeutische Alternative zur perkutanen Boostbestrahlung bei Patientinnen mit einem Mammakarzinom dar. Die Methode wurde im Mai 2007 in Kombination mit der adjuvanten perkutanen Radiatio im Brustzentrum des St. Elisabeth und St. Barbara Krankenhauses Halle (Saale) eingeführt. Das Ziel der Arbeit bestand darin, die Toxizität und das kosmetische Resultat nach intraoperativer Radiotherapie des Mammakarzinoms im Vergleich zur brusterhaltenden Operation mit ausschließlich perkutaner Bestrahlung zu analysieren. Die IORT-Gruppe umfasste 80 und die Kontrollgruppe 87 Patientinnen. Zwei Jahre post operationem war die Durchführung der IORT signifikant mit einem häufigeren Auftreten von Tumorbettindurationen und chronischen Schmerzen assoziiert. Das kosmetische Ergebnis wurde in der IORT-Gruppe subjektiv zu 78,3% als sehr gut und gut bewertet und in der Kontrollgruppe zu 87,7%. Aufgrund der geringgradigen Ausprägung der erfassten Nebenwirkungen und einer verringerten Lokalrezidivrate stellt die IORT eine interessante Therapieoption dar.
Intraoperative radiotherapy (IORT) using the Carl Zeiss AG Intrabeam® system is a novel therapeutic alternative to percutaneous boost irradiation on breast cancer patients. The method was introduced together with adjuvant percutaneous irradiation in the breast centre at the St. Elisabeth and St. Barbara hospital Halle (Saale), Germany, in May 2007. The aim of the study was to analyse the toxicity and cosmetic result of intraoperative radiotherapy applied to breast cancer compared to breast-conserving surgery using percutaneous irradiation only. The IORT and control groups included 80 and 87 patients, respectively. The IORT group showed a significantly higher occurrence of tumour bed induration and chronic pain two years after surgery; 78.3% of the IORT group rated the subjective cosmetic result as good or very good as compared to 87.7% in the control group. IORT represents an interesting therapeutic option due its low-grade side-effect profile and reduced local recurrence.
