Abstract: Ziel dieser Arbeit war die Evaluation der Lebervenenverschlussdruckmessung (HVPG) für der Prognose verschiedener Komplikationen der Leberzirrhose. Zuerst wurde die prognostische Bedeutung des HVPG zur Vorhersage einer Dekompensation und Entwicklung eines hepatozelluläres Karzinom bei kompensierter Leberzirrhose untersucht. Bei dekompensierter Leberzirrhose wurde die prognostische Rolle der HVPG zur Vorhersage des Todes untersucht. Eine zusätzliche Studie evaluiert, ob Einzelmessungen besser als wiedergeholte Messungen sind. Die Studien zeigten, dass der HVPG die Entwicklung verschiedener Komplikationen, sowie des hepatozellulären Karzinoms vorhersagen konnte. Zudem ist der prognostische Wert der Lebervenenverschlussdruckmessung zur Vorhersage des Todes demonstriert worden. Die Veränderung des HVPG gibt keine zusätzliche prognostische Information im Vergleich zur einzelnen basalen Messung. Zusammenfassend lässt sich sagen, dass der HVPG eine Rolle in der Vorhersage relevanter Komplikationen bei Zirrhose hat. Wiederholte Messungen scheinen keine zusätzliche prognostische Information zu haben.
The main pathophysiological mechanisms that lead to the complications of end-stage liver disease are portal hypertension and liver insufficiency. The aim is to evaluate the role of hepatic venous pressure gradient (HVPG) in the prognosis in cirrhosis. The ability of HVPG to predict decompensation and development of hepatocellular carcinoma was evaluated in compensated cirrhosis. In decompensated cirrhosis, the prognostic role of HVPG in the prediction of survival was studied. An additional study evaluated the information obtained from baseline values or repeat measurements. HVPG predicted the incidence of clinical decompensation and hepatocellular carcinoma. HVPG had a role in predicting survival in decompensated liver disease. The evaluation of the changes between measurements does not offer more information than the baseline measurement. In conclusion, HVPG has a role in prediction of relevant events of cirrhosis, especially in compensated cirrhosis. Repeat measurements do not offer more information than a baseline measurement.
