Abstract: Ziel der vorliegenden Arbeit war es retrospektiv zu eruieren welche Faktoren den Zeitpunkt des Wundverschlusses und das Auftreten einer non-in-sano-Resektion in der Basalzellkarzinomtherapie beeinflussen. Besondere Berücksichtigung fand dabei die Rezidiventwicklung. Im Untersuchungszeitraum vom 01.01.2004 bis zum 31.12.2007 ließen sich 309 Patienten mit 362 Primärtumoren und vier Rezidiven erfassen. Prospektiv wurde erstmals untersucht inwieweit der Zeitpunkt des Wundverschlusses mit der Lebensqualität der Basalzellkarzinompatienten korreliert. Dafür wurde ein Evaluationsbogen mit neun Fragen entwickelt, der sich im Untersuchungszeitraum 2013 an 57 Patienten ausgeben ließ. Retrospektiv ließ sich darstellen, dass die Exzisionswunde in mehr als 90 % der Fälle im Zuge des einzeitigen chirurgischen Verfahrens sofort verschlossen werden konnte. Die Gesamtrezidivrate betrug im Patientengut 1,1 % und wurde signifikant von einer unvollständigen Tumorentfernung beeinflusst. Prospektiv zeigte sich, dass 77 % der Patienten, bei denen sich der sofortige Wundverschluss anschloss, eine spätere Defektdeckung abgelehnt hätten.
The purpose of the present study was to elicit retrospectively the factors that affect the timing of wound closure and the occurrence of incomplete resection in the treatment of basal cell carcinoma. The development of recurrence was regarded particularly. This study collected 309 patients with 362 primary tumors and four recurrences during the investigation period from 01.01.2004 until 31.12.2007. Prospectively it was first examined to what extent the time of wound closure with the quality of life of patients with basal cell carcinoma correlated. For this reason an evaluation questionnaire with nine questions was developed. Total 57 patients could be interviewed during the investigation period in 2013. The retrospective study showed that the excision wound could be closed immediately in more than 90 % of cases with the one-stage surgery. The overall recurrence rate amounted 1,1 %. The incomplete resection of the tumor had a significant influence on the recurrence rate. Prospective 77 % of patients who were treated with the one-stage surgery would have rejected a later wound closure.
