Abstract: 6‐30 % der herzchirurgischen Patienten erleiden perioperativ ein Low‐Cardiac‐Output‐Syndrom. Bei Versagen der medikamtenösen Kreislauftherapie kommt u.a. die IABP zum Einsatz, deren wirklicher hämodynamischer Nutzen durch bisherige Studien nicht eindeutig geklärt werden konnte. In einer retrospektiven Beobachtungsstudie wurden Daten von 197 herzchirurgischen Patienten ausgewertet, die mit einer IABP behandelt wurden. Nach Auswertung des gesamten Patientenkollektivs erfolgte die Unterteilung der Patienten in Gruppen nach verschiedenen Gesichtspunkten wie Alter, BMI und Nierenfunktion. In der Gesamtgruppe kam es zu einer Steigerung des arteriellen Drucks und der linksventrikulären Auswurfleistung sowie einer leichten Senkung der Herzfrequenz und des Gefäßwiderstandes. Die Katecholamindosierungen konnten signifikant gesenkt werden. Bei den Subgruppen ergaben sich keine relevanten Unterschiede in den hämodynamischen Verläufen. Insgesamt zeigte sich ein eher geringer hämodynamischer Support durch die IABP für Patienten im Low‐Cardiac‐Output‐Syndrom.
6 to 30 per cent of the patients undergoing an open‐heart surgery perioperatively suffer from a low‐cardiac‐output‐syndrome.If medical treatment fails to reach a sufficient blood circulation, the intraaortic ballon pump is used. Its real haemodynamic benefit could not be sorted out clearly by former clinical studies. 197 patients undergoing cardiac surgery and treated with an iabp were part of this restrospective study. Beyond the analysis of the whole group of patients they were clustered into different cohorts depending on attributes like age, body mass index and renal function. Among the whole group of patients a rise of the arterial blood pressure and the left ventricular function as well as a slight drop of heart rate and vascular resistance could be recognized. The doses of catecholamines could be reduced after the iabp‐support was started. The analysis of the cohorts did not reveal any relevant differences in haemodynamic trends. Overall only a minor haemodynamic support attributable to the iabp for patients in perioperative low‐cardiac‐output‐syndrome could be found.
