Abstract: Vorliegende Arbeit untersucht die Vorhersage individueller Merkmale bei endogenen Psychosen durch Erkrankungs- und Verlaufscharakteristika. Als Repräsentanten endogener Psychosen wurden jeweils n=27 Patienten mit schizophrenen, bipolar schizoaffektiven und bipolar affektiven Störungen rekrutiert und deren individuelle Erkrankungs- und Verlaufscharakteristika sowie querschnittliche Aspekte erfasst. In den Ergebnissen fanden sich bezüglich aller individuellen Merkmale deutlich kleinere Varianzaufklärungen durch die kategorialen Diagnosen (0-10%) als durch die individuellen Erkrankungs- und Verlaufscharakteristika (13-46%). Die Ergebnisse sprechen für eine stärkere Berücksichtigung des klinischen Verlaufs neben der kategorialen Diagnose, wenn es um Entscheidungen bezüglich Therapie und Prognose geht.
The study presented examines the predictability of current characteristics of endogenous psychoses by features of the individual course of the illness. Endogenous psychoses were represented by samples of n=27 subjects each of patient groups suffering from schizophrenia, bipolar schizoaffective disorders and bipolar mood disorders. Features of the individual course of the illness were assessed as well as current characteristics in terms of mood and psychotic symptoms, cognition, level of functioning, and quality of life. Results showed that in prediction of all current characteristics categorial diagnosis explained less variance (0%-10%) than features of the individual course of the illness (13-46%). These results demand a stronger consideration of clinical course besides the categorial diagnosis in decisions for therapy and prognosis.
