Abstract: Betulinsäure (BA) wurde als spezifischer, Apoptose-induzierender Inhibitor des malignen Melanoms beschrieben, einer Krebserkrankung mit weltweit steigender Inzidenz. In dieser Arbeit wurde die Reaktionen verschiedener epidermaler humaner Zellsysteme (normale humane epidermale Melanozyten (NHEM) und Keratinozyten (NHEK)) im Vergleich zu einer maligne entarteten Melanom-Zelllinie (IGR1) untersucht, um die Spezifität der BA-Wirkung bei einer topischen Anwendung zu verifizieren. Die Wirkung der BA kann nicht als spezifisch für das maligne Melanom bewertet werden, da eine toxische Wirkung und eine Induktion der Apoptose durch BA in allen Zellkulturen im Zeitintervall 12 - 72 h nachgewiesen werden konnte. Mit dem APO2.7-Test konnte nach 48 h und 5 µgxml-1 BA in allen Zelllinien ein signifikanter Anstieg der APO2.7-positiven Zellen aufgezeigt werden. Eine DNA-Fragmentierung ist in allen Zellsystemen durch die TUNEL-Methode am Durchflusszytometer detektierbar. Caspasen spielen eine wesentliche Rolle bei der durch BA vermittelten Apoptose, erkenntlich am Anstieg M30-positiver Zellen. Eine Hemmung der Caspasen durch den Inhibitor Z-VAD-FMK führt zu einem Rückgang der DNA-Schnittprodukte im TUNEL-Test, nicht aber zu einer Erhöhung der Anzahl vitaler Zellen im Annexin-V/PI-Test. P53 ist nicht in eine BA-induzierten Apoptose involviert. Normale Keratinozyten reagieren auf BA nur zu einem geringen Maß mit einer Phosphatidylserinumlagerung und Verlust der Membranintegrität aber mit einem starken Anstieg von Stoffwechselvorgängen der epidermalen Differenzierung. Eine Stimulation der terminalen Differenzierung ist ab 2 µg/ml BA anhand des Anstiegs des Involucringehaltes in NHEK nachweisbar. Durch BA (8 µg ml-1) wird, wie durch eine 2,5 mM Ca2+-Positivkontrolle, eine Expression von Filaggrin induziert. Insgesamt liefern die vorliegenden Ergebnisse Hinweise, dass die Toxizität der BA nicht auf neuroektodermale Zellen beschränkt bleibt und eine systemische Anwendung vorerst auszuschließen ist. Die Ergebnisse geben Ansatzpunkte, BA nach Modifikationen der Molekülstruktur und Erhöhung der Spezifität, für die topische Behandlung epidermaler Störungen nach UV-Exposition, bei Pigmentstörungen (z.B. Lentigines), Differenzierungsstörungen und/oder epidermalen Hyperproliferationen (z.B. Psoriasis) heranzuziehen.
Betulinsäure (BA) is described as a selective, apoptosis-induced inhibitor of the malign Melanoma, a cancer illness with increasing incidence world-wide. This work compares the reaction of different epidermal humane cell systems (normal humane epidermal melanocytes (NHEM) and keratinocytes (NHEK) and a malign melanoma cell line (IGR1)) to verify the BA-effects during a topical application. The effect of the BA can not be weighted as melanoma specific, because a toxic effect and an induction of apoptosis by BA were proven in all cell cultures in the time interval 12 - 72 h. With the APO2.7-test a significant increase of the APO2.7 positive cells was shown after 48 h and 5 µg BA in all cell lines. DNA-fragmentation was detected in all cell systems by the TUNEL-method using flow cytometry. Caspases are involved in BA-mediated apoptosis. This shows an increase of M30 positive cells. Apoptosis, especially DNA-Fragmentation, was blocked by the broad-range peptide inhibitor of caspases ZVAD-FMK, but there was no increase of vital cells in the Annexin V/PI Test. The epidermal cells show a p53-independent apoptosis. Normal keratinocytes react to BA only at a low measure with a phosphatidylserin-translocalisation and loss of the membrane integrity, but with a strong increase of metabolism events of the epidermal differentiation. A stimulation of the terminal differentiation is verifiable as of 2µg/mL BA by increasing the involucrin content in NHEK. One expression of Filaggrin can be induced using BA (8µg/mL) like a positive control (2.5mM Ca2+). In sum, the results show the toxicity of the BA does not remain restricted to neuroectodermal cells and therefor a systemic application should be excluded for the time being. The results are starting-points for the use of BA, after modifications of the BA-molecule structure and by raising the selectivity, for the topical treatment of epidermal diseases after UV-exposure, pigment disturbances (e.g. lentigines), keratinocyte differentiation and/or epidermal hyperproliferation disorders (e.g. psoriasis).
