Title: Optimierung einer Kombinations-Chemotherapie mit Docetaxel, Gemcitabin und Oxaliplatin bei Patienten mit fortgeschrittenen soliden Tumoren

Abstract: Zielsetzung: Das Ziel dieser Therapieoptimierungsstudie war die Bestimmung der maximal tolerablen Dosis und der dosislimitierenden Toxizität der Kombination Docetaxel-Gemcitabin-Oxaliplatin bei Patienten mit fortgeschrittenen soliden Tumoren. Methoden: 41 Patienten wurden erfasst und mit einer 60-Minuten-Infusion Docetaxel gefolgt von einer 30-Minuten-Infusion Gemcitabin an Tag 1 und mit einer 90-Minuten-Infusion Oxaliplatin an Tag 2 (Wiederholung alle 14 Tage) nach folgendem Dosiseskalationsschema behandelt: Stufe 0 35, 800 und 65 mg/m², Stufe I 45, 800 und 65 mg/m², Stufe II 45, 1000, 65 mg/m², Stufe III 55, 1000, 65 mg/m², Stufe IV 55, 1300, 65 mg/m², Stufe V 55, 500, 70 mg/m² und Stufe VI 55, 500, 80 mg/m². Ergebnisse: Insgesamt erfolgten 276 Gaben. Die dosislimitierende Toxizität wurde auf Stufe VI erreicht und stellte Diarrhoe dar. Diarrhoe vom Grad 3 trat bei 3 von 5 Patienten der höchsten Dosisstufe auf. Diarrhoe vom Grad 1 - 2 trat bei 13 (32 %) Patienten auf. Die hämatologische Toxizität war nie dosislimitierend, trat aber häufig auf: Anämie Grad 1 - 3 bei 35 (85,4 %) und Leukopenie Grad 1 - 3 bei 20 (48,7 %) Patienten. 6 Episoden einer febrilen Neutropenie wurden registriert, wobei hämatopoetische Wachstumsfaktoren nicht routinemässig eingesetzt worden waren. Weitere häufige Toxizitäten waren Asthenie, Übelkeit und Erbrechen, Neurotoxizität und Hauttoxizität. Die objektive Remissionsrate betrug 28,2 %. Eine Patientin mit Ovarialkarzinom erzielte eine komplette Remission von 9,5 Monaten Dauer. 10 Patienten erzielten partielle Remissionen. Dabei handelte es sich um HNO-Karzinome (3 von 14), Pankreaskarzinom (1 von 4), Ovarialkarzinome (2 von 5), Weichteilsarkom (1 von 4), CUP (1 von 3), und Bronchialkarzinom (1 von 4). Schlussfolgerung: 55 mg/m² Docetaxel, 500 mg/m² Gemcitabin und 70 mg/m² Oxaliplatin ist eine machbare und sichere Kombinationschemotherapie für Patienten mit fortgeschrittenen soliden Tumoren. Die beobachtete Antitumoraktivität verdient Beachtung und muß in Phase II-Studien bei Ovarial- und Pankreaskarzinomen weiter evaluiert werden. Außerdem sind weitere Untersuchungen zur Kombination mit Strahlentherapie erforderlich.
Rationale: to find the maximal tolerable dose and dose limiting toxicity of the combination docetaxel/gemcitabine/oxaliplatin in patients with advanced solid tumours. Patients and methods: fourty-one patients were enrolled and treated with docetaxel (1-h infusion), gemcitabine (30-min infusion) on day 1 followed by oxaliplatin (90-min infusion) on day 2 in a biweekly (q15d) outpatient setting after the following dose escalation scheme: level 0 35, 800 und 65 mg/qm, level I 45, 800 und 65 mg/qm, level II 45, 1000, 65 mg/qm, level III 55, 1000, 65 mg/qm, level IV 55, 1300, 65 mg/qm, level V 55, 500, 70 mg/qm and level VI 55, 500, 80 mg/qm. Results: Total number of cycles was 276. Dose limiting toxicity was diarrhea and was reached on level VI. Diarrhea of grade 3 (WHO-criteria) occurred in 3 of 5 patients of the highest dose level. Diarrhea of grade 1-2 occurred in 13 (32 %) patients. Myelosuppression never was dose limiting but occurred frequently: anemia of grade 1 - 3 in 35 (85.4 %) and leucopenia grade 1 - 3 in 20 (48.7 %) patients. 6 episodes of febrile neutropenia occurred. Hematologic growth factors were not used routinely. Other frequently observed toxicities were asthenia, nausea and vomiting, neurotoxicity and skin toxicity. Objective response was 28.2 %. One patient with ovarian cancer had a CR of 9.5 months. 10 patients had a PR (3 of 14 head and neck carcinoma; 1 of 4 pancreatic carcinoma, 2 of 5 ovarian carcinoma, 1 of 4 soft tissue carcinoma, 1 of 3 CUPs, and 1 of 4 lung carcinoma). Conclusion: 55 mg/qm docetaxel, 500 mg/qm gemcitabine and 70 mg/qm oxaliplatin is a feasible and safe combination with a promising activity for patients with advanced solid tumours. This regime has to be further evaluated in phase II studies with ovarian and pancreatic cancer and in combination with radiotherapy.
