dc.description.abstractDie Nierenentwicklung nach operativer Therapie bzw. extrakorporaler Stoßwellenlithotripsie (ESWL) wurde untersucht. Es wird über 67 Kinder mit Harnsteinleiden berichtet. Knaben waren 1,5 mal häufiger betroffen als Mädchen. Im Rahmen der Diagnostik wurden bei ¼ der Patienten Fehlbildungen des harnableitenden Systems gefunden. 92% der Steine konnten sonographisch diagnostiziert werden. Bis 1987 erfolgte die Therapie abgesehen von konservativen Versuchen ausschließlich operativ. Nach Einführung der ESWL konnten danach zunehmend auch kleinere Kinder nichtinvasiv behandelt werden. Die Operation verlor jedoch wegen der gleichzeitigen Korrektur begleitender Fehlbildungen des harnableitenden Systems nicht an Bedeutung. Die Nachuntersuchung erfolgte bei 38 Kindern sonographisch und szintigraphisch (MAG 3) im Durchschnitt 6 1/3 Jahre nach Therapie. So konnten sowohl die morphologische als auch die funktionelle Entwicklung der betroffenen Niere beurteilt werden. Im Ergebnis war die Nierenentwicklung der Kinder mit assoziierter Fehlbildung des harnableitenden Systems deutlich schlechter. Die Behandlungsmethode dagegen hatte keinen Einfluß auf die Nierenentwicklung.-
dc.description.abstractThe objective of this work was to compare kidney development following surgical therapy or extracorporeal shock wave lithotripsy (ESWL) in paediatric patients with urolithiasis. The records of 67 patients are reviewed for which the incidence of urolithiasis was 1.5 times higher in boys than in girls. In 25 % of the patients malformations of the urinary tract were identified. 92 % of the kidney stones were detected using ultrasonography. Until 1987 all patients underwent surgical therapy, except for some conservative attempts of treatment. After its introduction ESWL raised the possibility of a noninvasive treatment in small children. Nonetheless conventional surgery did not loose its importance due to the possibility of an intraoperative correction of malformative uropathies. The follow-up was conducted in 38 patients using ultrasonography and scintigraphy (MAG 3) approximately 6.3 years after therapy. Kidney development was analysed in terms of morphological alterations as well as renal function. The state of development was considerably lower in patients with associated malformations. However the choice of treatment did not have an influence on kidney development.eng
