dc.description.abstractDie chirurgische Resektion sämtlicher Tumorzellen ist die einzige kurative Therapie beim medullären Schilddrüsenkarzinoms (MTC). Über das erforderliche Ausmaß der Lymphknotendissektion wird kontrovers diskutiert. In dieser prospektiven Arbeit wurden die Daten von 65 erstoperierten Patienten mit histologisch gesichertem MTC analysiert. Für alle Patienten erfolgte standardisiert eine totale Thyreoidektomie mit beidseitiger zentraler und lateraler Lymphknotendissektion mit intraoperativem Pentagastrin-Stimulationstest (IOPST) nach Dissektion des jeweiligen Kompartimentes. Bezugnehmend auf das chirurgische Vorgehen wurden die Patienten in 2 Gruppen unterteilt; zentrifugal (ZF) und zentripetal (ZP). In der ZF-Gruppe zeigte der IOPST mit einer Stimulierbarkeit von 115% einen negativen Vorhersagewert von 96% in Bezug auf das noch Vorhandensein von tumorbefallenen Lymphknoten in situ. Die Ergebnisse des IOPST prädizieren das erforderliche Ausmaß der Lymphknotendissektion der lateralen Kompartimente beim zentrifugalen Vorgehen ausreichend zuverlässig, um das Resektionsmaß theoretisch danach auszurichten.ger
dc.description.abstractSurgical resection of all tumor cells is the only curative therapy for medullary thyroid carcinoma (MTC). The extent of lymph node dissection required is, however, controversial. In this study data from 65 patients with primary surgical therapy with MTC were analyzed retrospectively. All patients underwent a total thyroidectomy with bilateral central and lateral lymph node dissection with intraoperative Pentagastrin-stimulation test (IOPST) after dissection of the corresponding compartment. Regarding the surgical procedure, the patients were divided into 2 groups; centrifugal (ZF) and centripetal (ZP). In the centrifugal group, the IOPST with a stimulability of 115% presented a negative predictive value of 96% with respect to the presence of tumor-related lymph nodes in situ. The results of IOPST predicted the required extent of lymph node dissection of the lateral compartments in the centrifugal procedure theoretically with adequate reliably.eng
