Abstract: Hintergrund: Ziel war es, die korneale Wundheilung nach intrastromaler Femtosekunden (fs)-Laser-Keratotomie zu untersuchen. Ferner sollte bestimmt werden, ob durch intrastromale Photodisruption eine Dickenabnahme und Abflachung der Hornhaut erreicht werden kann. Material und Methoden: Kaninchen erhielten eine fs-Laser-assistierte Keratotomie oder Keratektomie. Die Hornhäute wurden mittels Spaltlampe, Pachymetrie, Keratometrie, in vivo Konfokal-, Licht-, Fluoreszenzmikroskopie und quantitativer RT-PCR untersucht. Ergebnisse: Die fs-Keratotomie führte zu einer hypozellulären stromalen Narbe. Sowohl eine Ablagerung von Fibronektin und Tenascin als auch der Zelltod und die Proliferation von Keratozyten wurden beobachtet. Eine Differenzierung von Keratozyten in Thy-1- oder α-SMA-positive Fibroblasten konnte nicht detektiert werden. Die TGF-β1-mRNA und -Protein-Expression stieg nicht signifikant an. Sechs Monate nach fs-Keratektomie fand sich eine Abnahme der Hornhautdicke (−36.3±6.9 µm) und der Hornhautradien (−0.21±0.10 mm). Diskussion. Eine isolierte stromale Verletzung mittels fs-Laser geht mit einer günstigen Wundheilungsreaktion und einem Transparenzerhalt der Hornhaut einher. Eine Dickenabnahme der Hornhaut kann durch intrastromale Gewebeablation zuverlässig erreicht werden.
Purpose: To examine the corneal repair response after intrastromal femtosecond (fs) laser keratotomy and to determine whether corneal thinning and flattening can be achieved by intrastromal photodisruption. Methods: Rabbits underwent keratotomy or keratectomy performed with an fs laser. Corneas were evaluated using slit lamp, pachymetry, keratometry, in vivo confocal-/ light-/ fluorescence-microscopy and quantitative RT-PCR. Results: Fs keratotomy resulted in a hypocellular stromal scar. Deposition of fibronectin and tenascin as well as death and subsequent proliferation of keratocytes were observed. No differentiation of keratocytes into Thy-1- or α-SMA-positive fibroblasts could be detected. TGF-β1-mRNA and -protein expression did not significantly increase. Six month after fs keratectomy corneas showed a decrease of thickness (−36.3±6.9 µm) and of corneal radii (−0.21±0.10 mm). Conclusions. Isolated stromal injury using an fs laser is associated with a favorable wound healing response preserving corneal transparency. Corneal thinning can be reliably achieved using intrastromal tissue modeling with a fs laser.
