Abstract: Die intraaortale Ballongegenpulsation (IABP) ist aktuell bei Patienten im infarktbedingten kardiogenen Schock (IkS) das am häufigsten angewandte mechanische Unterstützungssystem. Trotz eingeschränkter Evidenz zur therapeutischen Überlegenheit der IABP bei Patienten mit IkS gibt es eine Klasse-I-Empfehlung in den amerikanischen und europäischen Leitlinien. Ziel dieser systematischen Übersichtsarbeit war die Beurteilung von Effektivität und Sicherheit der IABP bei Patienten mit IkS bezugnehmend auf Mortalität. Hierzu wurden per elektronischer Datenbankrecherche insgesamt 6 randomisiert klinische Studien und ein laufendes Projekt ermittelt. Hier wurde die IABP-Therapie einer Standardbehandlung ohne IABP oder einer Therapie mit alternativen Herzunterstützungssytemen gegenübergestellt. Die Ergebnisse der Metaanalyse von insgesamt 190 Patienten mit IkS zeigen trotz nachweislicher Verbesserung der Hämodynamik bei IABP-Einsatz für keine der Behandlungsgruppen einen Überlebensvorteil auf.
Intra-aortic balloon pump (IABP) is currently the most commonly used mechanical assist device for patients with cardiogenic shock (CS) due to acute myocardial infarction (AMI). Although there is only limited evidence by randomised controlled trials, the current American and European guidelines strongly recommend the use of IABP in patients with infarction-related CS. To determine the effect of IABP versus non-IABP or other assist devices in terms of efficacy and safety in patients with AMI complicated by CS, searches of medical literature databases, registers of ongoing trials were conducted. 6 eligible and one ongoing studies were identified. Data from a total of 190 patients with acute myocardial infarction and CS were included in the meta-analysis of the systematic review. Available results suggests that IABP may have a beneficial effect on the haemodynamics, however there is no convincing randomised data to support the use of IABP in infarct related CS.
