dc.description.abstractFür die Patientenzufriedenheit und die Funktionalität von doppelkronenverankertem Zahnersatz ist die Gesamtretention von essentieller Bedeutung. Ziel dieser in-vitro-Studie war neben der Entwicklung einer Prüfapparatur, die Quantifizierung des Retentionsverhaltens von Doppelkronen [DK] mit und ohne Friktionsstift [FS] aus jeweils zwei verschiedenen Materialkombinationen. Unter annähernd klinischen Bedingungen wurden Doppelkronenpaare in einer Prüfprothese integriert und einer artifiziellen Alterung unterzogen (50000 Kauzyklen, 15000 Ein- und Ausgliederungen). Die initial mittleren Gesamtretentionen lagen bei den edelmetallfreien DK mit FS bei 14 N, bei den baugleichen ZrO2-Primärkronen und edelmetallfreien Sekundärkronen mit Gold-FS bei 12,2 N. Die edelmetallfreien Teleskope erzielten initial 12,5 N. Hochsignifikant niedriger lag mit 9,6 N der Wert der edelmetallhaltigen Teleskope (p<0,001). Die künstliche Alterung bewirkte bei den Teleskopen (MWT:66,5%) und bei den edelmetallfreien DK mit FS (28 %) einen hochsignifikanten Retentionskraftverlust (p<0,001). Bei den ZrO2-DK zeigte sich keine Auswirkung auf das Retentionsverhalten (p=0,857). Zusammenfassend kann konstatiert werden, dass sich die neu entwickelte Prüfapparatur bewährt hat und die klinische Anwendung der DK mit FS empfohlen werden kann.-
dc.description.abstractPatient’s satisfaction and adequate function of removable partial dentures highly depend on the retention of the prosthesis. The purpose of this in vitro study was to develop a special testing device and to compare the retention forces of double crowns with and without friction pins. Under almost clinical conditions, double crowns made of different materials were integrated in a simulated removable partial denture and artificially aged (50 000 chewing cycles, 15 000 insertion-removal cycles). The initial mean retention forces were 14 N for non-precious metal telescopes with friction pins, 12.2 N for zirconium/non-precious metal telescopes with gold friction pins and 12.5 N for non-precious metal telescopes. Precious metal telescopes showed highly significant lower retention forces of 9.6 N (p<0.001). Compared to conventional precious metal telescopes (mean value 66.5 %), non-precious metal telescopes with friction pins (28 %) showed a lower decrease of retention force after artificial aging (p<0.001). Whereas, the retention force of Zirconium/non-precious metal telescopes were not effected by aging (p=0.857). Within the limits of this in vitro study, it can be concluded that the testing device is an approved method to simulate retention forces and that the clinical use of double crowns with friction pins can be recommended.eng
