Abstract: Hintergrund: ERCC1 ist eine Nuklease, welche eine wesentliche Rolle bei der Nukleotid-Exzisions-Reparatur spielt. In mehreren Studien konnte ERCC1 als prädiktiver Marker für eine platinbasierte Chemotherapie identifiziert werden. Das Ziel dieser Arbeit war es zu untersuchen, ob es Verteilungsunterschiede von ERCC1 auf prognostisch relevante Faktoren des Bronchialkarzinoms gibt. Methoden: Untersuchungsgut waren 153 Patienten, bei denen in den Jahren 2008 und 2009 eine operative Bronchialtumorentfernung durchgeführt wurde. Dabei wurde das Resektat standardmäßig immunhistochemisch auf ERCC1 untersucht. Die Angaben bezüglich der prognostisch relevanten Faktoren wurden aus den Patientenakten entnommen. Bei der Datenauswertung kamen die Darstellungen absoluter und relativer Häufigkeiten inklusive der 95-prozentigen Konfidenzintervalle zur Anwendung. Ergebnisse: Es konnte gezeigt werden, dass vor allem Plattenepithelkarzinome deutlich häufiger ERCC1-positive Befunde aufwiesen als andere Histologien. Von Stadium I bis IIIA verringerte sich der Anteil ERCC1-positiver Tumore. Beim Raucherstatus, Alter und Geschlecht konnten nur geringfügig bis keine Verteilungsunterschiede festgestellt werden. Fazit: Aufgrund der niedrigen Fallzahl konnten in dieser Arbeit lediglich Trends aufgezeigt, jedoch keine zweifelsfreie Verteilungsunterschiede von ERCC1 auf prognostisch relevante Faktoren des Bronchialkarzinoms nachgewiesen werden.
Background: ERCC1 is a nuclease, which plays a key role in the nucleotide-excision-repair-mechanism. In several studies ERCC1 could be identified as a predictive marker for a platinum-based chemotherapy. The objective of this dissertation is to examine, if there are differences in the distribution of ERCC1 on prognostic relevant factors of lung cancer. Methods: The objects of this study were 153 patients, who had a lung cancer extirpation in the years 2008 and 2009. Following the standard procedure, the resected tumor has been tested for ERCC1 immunohistochemically. The information about the prognostic relevant factors was found in the patient files. For the distribution analysis, the absolute and relative frequency, including the 95% confidence intervals, were used. Results: It could be shown that with the squamous cell carcinomas have the highest rate of ERCC1-positive findings in comparison with other tumor histologies. Additionally the number of ERCC1-positive findings decreased from stage I to stage IIIA. Only minor differences were found in the smoking status, sex and age. Conclusion: Due to the low case counts, only trends can be shown. In conclusion, the evidence is not sufficient, however, to ascertain that there are irrefutable differences in the distribution of ERCC1 on prognostic relevant factors of lung cancer.
