Abstract: Ohne weitere Effizienzsteigerungen sind Krankenhäuser in Deutschland langfristig weder dem finanziellenDruck noch den Herausforderungen des demographischen Wandels gewachsen. Zur Unterstützung von Ressourcenkapazitätsentscheidungen auf strategischer, taktischer und operativer Ebene wird in dieser Dissertation ein datengetriebenes generisches Krankenhausentscheidungsunterstützungssystems konzipiert.Es besteht aus den Modulen Daten- und Prozess-Mining sowieOptimierung und Simulation. Im Rahmen dieser Module werden konkrete Werkzeuge zur automatischen Gewinnung planbarer klinischer Pfade für elektive Behandlungen, zur automatischen Planung vollständiger elektiver Behandlungen sowie zur Evaluation von Veränderungen im Einsatz krankenhausinterner Ressourcen entwickelt. Alle Werkzeuge werden anhand praxisnaher Fallstudien unter Betrachtung von Mikrostrukturen und unter Verwendung nach § 21 KHEntgG standardisierter Krankenhausdaten implementiert und erfolgreich evaluiert.
Without further enhancements in resource efficiency, German hospitals are not capable of meeting the future financial and patient-centered care challenges..This thesis proposesa concept for a generic data driven decision support system for strategical, tactical and operational resource capacity panning problems for German hospitals. The system consists of three linked modules: Data- and process mining, optimization and simulation. Furthermore, integrated tools to automatically obtain scheduling-focused clinical pathways for elective patients, to automatically schedule the entire treatment process of elective inpatients and to evaluate changes in handling clinical resources are developed. All tools are implemented and successfully evaluated on the basis of practical case studies, respecting microstructures of clinical processes and resource allocation as well as using standardized clinical data corresponding to § 21 KHEntgG.
