Abstract: Ziel der Studie war es, die Anwendbarkeit und Wirksamkeit der SVC-Stenting bei der Behandlung vom Vena cava superior-Syndrom bei Krebspatienten zu beurteilen. Die Studie umfasste 95 Fälle mit VCS Obstruktion durch histologisch gesicherten malignen Tumoren. 120 SVC-Stents wurde in dem Zeitintervall zwischen 05/1996 und 11/2009 implantiert. In unserer Studie konnten wir eine klinische Erfolgsrate von 86,3% erreichen. Wir haben verschiedene Stenttypen eingesetzt und konnten keine signifikante Korrelation zwischen der Rate von Komplikationen und den Stent-Typ finden. Wir haben versucht die Rolle der SVC-Stents wie folgt zu definieren: Eine obere Einflussstauung bei neu diagnostizierten NSCLC und in Fällen, die schlecht auf eine Chemotherapie ansprechen, indiziert eine Stentimplantation. Bei neu diagnostiziertem SCLC mit SCVO besteht primär die Indikation zur Chemo- und/oder Strahlentherapie. Wenn darunter keine Rückbildung der Symptomatik eintritt, ist Stenting indiziert. Stenting ist die Therapie der Wahl beim Auftreten eines SCVS unter einer Chemo- und / oder Strahlentherapie. Ein akutes SCVS bei Verdacht auf eine maligne thorakale Raumforderung indiziert eine Stentimplantation auch vor einer histologischen Tumorsicherung.
The aim of the study was to evaluate the applicability and efficacy of SVC stenting in the treatment of superior vena cava syndrome in cancer patients. The study included 95 cases with VCS obstruction by histologically proven malignant tumors. 120 SVC stents were implanted in the time interval between 05/1996 and 11/2009. In our study, we achieved a clinical success rate of 86.3%. We used different stent types and could not find any significant correlation between the rate of complications and the stent type. We have attempted to define the role of SVC stents as follows: Upper congestive stasis in newly diagnosed NSCLC and in cases poorly responsive to chemotherapy indicates stent placement. In newly diagnosed SCLC with SCVO, the primary indication is chemo- and / or radiotherapy. If there is no regression of the symptoms, stenting is indicated. Stenting is the treatment of choice for the occurrence of SCVS under chemo- and / or radiotherapy. An acute SCVS in suspected malignant thoracic mass indicates a stent implantation before a histological tumor proof.
