dc.description.abstractMittels einer z.Z. in der Immunhistochemie etablierten Färbemethode wurde anhand zytologischer Präparate überprüft, ob die Aussagekraft der Routinezytologie in Kombination mit dieser immunzytochemischen Färbung zur Zytokeratindarstellung (APAAP-Methode) an Zytologiepräparaten aus pulmonalen Prozessen im Vergleich zur Histologie gesteigert werden kann. Nach Gewinnung zytologischer Präparate (OP-Präparate, Katheterbiopsie, Feinnadelpunktion) von 84 Patienten aus pulmonalen Prozessen wurden zunächst Giemsa-Präparate hergestellt. Weiterhin wurde ein anderer Teil der Nativpräparate der immunzytochemischen Färbung zur Darstellung von CK 5/6, CK 7, CK 20 und einem CK-Komplex (CK 5, CK 6 und CK 18) unterzogen und im Verlauf ausgewertet. Nach Feststellung der jeweiligen Zytokeratinmuster konnte sich unter Zuhilfenahme einer Vergleichstabelle eine Tumortypenbestimmung anschließen. Retrospektiv wurden als Referenz dazu die entsprechenden Histologiebefunde verwendet. Zusammenfassend stellte sich in Bezug auf die Dignitätsbeurteilung der vorliegenden Befunde ein positiver prädiktiver Wert von 0,986 der Routinezytologie ein. Das primäre Adenokarzinom der Lunge wurde zytologisch in 53,85% der Fälle eindeutig erkannt und die Aussagekraft durch das spezielle Zytokeratinmuster auf 80,77% der Fälle verbessert. Im Hinblick auf das pulmonale Plattenepithelkarzinom lag die zytologische Diagnostik bei nahezu 74%, ohne dass eine eindeutige Effektivitätssteigerung durch die Zytokeratinfärbung verzeichnet werden konnte. Auch in der Diagnostik des undifferenzierten großzelligen Karzinoms, des anaplastisch kleinzelligen Karzinoms, anderer seltener Tumore und pulmonaler Metastasen konnte die immunzytochemische Färbung nicht überzeugen. Insgesamt führte die hier angewandte Darstellung der Zytokeratine in der Diagnostik pulmonaler Prozesse lediglich beim Adenokarzinom zu neuen Erkenntnissen. Wegen enormer Materialkosten und des hohen Zeitaufwandes des Färbeprozesses ist diese Methode als routinemäßige Ergänzung zur konventionellen Zytologie nicht zu empfehlen.-
dc.description.abstractUsing a method to stain cytokeratins on cytological preparations from pulmonary processes currently only established in immunohistochemistry it was examined, if the significance of clinical cytology in combination with this method (APAAP-method) compared to histology can be increased. After getting cytological specimens from pulmonary processes of 84 patients (smears taken by surgery, catheter biopsy, fine needle aspiration) a conventional Giemsa staining was applied and then evaluated. Furthermore, commercial monoclonal antibodies to detect CK 5/6, CK 7, CK 20 and a CK-complex (CK 5, CK 6 and CK 18) were employed. According to the expression of cytokeratin the respective tumour type was defined. Retrospectively histology findings were taken in reference. In summary, the positive predictive value of conventional cytology concerning evaluation of dignity added up to 0.986. The primary adenocarcinoma of the lung was clearly identified by conventional cytology in 53.85% and the additional use of cytokeratin immunostaining led to diagnosis in 80.77% of the cases. With regard to the pulmonary squamous cell carcinoma, cytological findings nearly came to 74% without an explicit increase of efficiency by cytokeratin staining. Immunocytochemical staining was not convincing in the diagnosis of undifferentiated large cell carcinoma, the SCLC, other rare tumours and pulmonary metastases as well. Overall, the presentation of cytokeratins in cytological preparations only led to new findings concerning the diagnosis of pulmonary adenocarcinoma. Due to enormous material costs and the high expenditure of time this method of staining cytokeratins is not recommended as a routine complement to conventional cytology at all.eng
