dc.description.abstractDurch eine retrospektive Analyse wurden mittelfristige Ergebnisse nach Implantation der Genesis-II-Knietotalendoprothese ausgewertet. Dabei erfolgte die Gegenüberstellung von Standardfemurkomponenten aus einer Kobalt-Chrom-Legierung und Femurkomponenten aus einer oberflächenkeramisierten Zirkonium-Niob-Legierung bei gleichem Prothesendesign. In den Jahren 2000 bis 2002 wurde bei 40 Patienten eine Knietotalendoprothese des Typs Genesis II mit oberflächenkeramisierter Femurkomponente aus einer Zirkonium-Niob-Legierung zementiert implantiert. Von diesen Implantationen konnten 31 klinisch und radiologisch in einem mittleren Zeitraum von 46,4 Monaten nachuntersucht werden. Die klinischen und funktionellen Ergebnisse wurden durch den Score der Knee Society (Insall et al. 1989) und den WOMAC-Index erfasst. Die radiologische Auswertung erfolgte nach dem Roentgenographic Evaluation and Scoring System der Knee Society. Ziel der Untersuchungen war die Darstellung der mittelfristigen klinischen und röntgenologischen Ergebnisse dieser Patienten im Vergleich zu einer identisch nachuntersuchten Patientengruppe bei denen die Implantation von Standardfemur-komponenten des gleichen Systems erfolgte. Die Auswertung der Ergebnisse erbrachte keine statistisch signifikanten Unterschiede zwischen den Patientengruppen bei den genannten klinisch-funktionellen Parametern. Bei der Auswertung der röntgenologischen Ergebnisse fanden sich keine signifikanten Unterschiede hinsichtlich der Implantatstellung, die in beiden Gruppen mit den Forderungen aus der Literatur übereinstimmt. Die statistische Auswertung der gefundenen Resorptionssäume ergab signifikante Unterschiede zwischen beiden Gruppen. Dabei wurde ein vermehrtes Auftreten von Resorptionssäumen beim Gelenkersatz mit Zirkonium-femurkomponenten nachgewiesen.-
dc.description.abstractMid-term results after implantation of the Genesis-II-TKR were evaluated in an retrospective study. Thereby a comparison between standard femur components of a CoCr alloy and identical designed components of a ceramic surfaced oxidized ZrNb alloy was made. In the years 2000 until 2002 the Genesis-II-TKR with the ceramic surfaced ZrNb femur component had been implanted at 40 patients. 31 of these implantations could be evaluated clinically and radiologically within a medium period of 46.4 months. The clinical and functional results were documented by the Score of the Knee Society (Insall et al. 1989) and the WOMAC index. The radiological evaluation was done due to the Roentgenographic Evaluation and Scoring System of the Knee Society. The aim of this study was to demonstrate medium clinical and radiographical results on these patients in comparison to an identical group of patients who had been implanted with standard femur components of the same system. The evaluation of the results did not show any statistically significant differences between these two groups of patients at the mentioned clinically functional parameters. No significant differences regarding the orientation of the components, that complies in both groups with the requirements in literature, could be found during the evaluation of the radiographical results. The statistic evaluation of the found radiolucent lines showed significant differences between both groups. An increased occurance of radiolucent lines could be demonstrated for the joint replacements using the ceramic surfaced ZrNb femur components.eng
