Abstract: Von Januar 2002 bis April 2003 wurden an der Urologischen Universitätsklinik Halle die Daten von 56 Patienten prospektiv und nicht randomisiert erfasst, die wegen eines lokal begrenzten Prostatakarzinoms einer radikalen Prostatovesikulektomie unterzogen wurden. Jeweils 28 Patienten wurden mittels retropubisch offener radikaler RPVE bzw. laparoskopisch extraperitonealer RPVE operiert. Es gab keine signifikanten Unterschiede bezüglich Alter (65,3 Jahre vs. 62,4 Jahre) und BMI (27,6 vs. 25,8). Die durchschnittliche Operationszeit war mit 144 Minuten für die offene RPVE signifikant länger als für die laparoskopische RPVE mit 220 Minuten (p Für weitere Parameter wie intraoperativen Blutverlust (p 0,422), postoperativen PSA-Wert (p 0,197), Verweildauer des Harnblasenkatheters (p 0,837) und Hospitalisationsdauer (p 0,262) ergaben sich keine statistisch signifikanten Unterschiede. Die Gesamtmorbidität betrug in der Gruppe der offenen RPVE 21,4% und in der laparoskopischen Gruppe 17,9% bei 0% Gesamtmortalität. Keine statistisch signifikanten Unterschiede zeigte auch die postoperative Stadieneinteilung der Tumoren wie auch die Raten an R0 und R1 Resektionen in beiden Patientengruppen. Die Ergebnisse der vorliegenden Vergleichsstudie unterstreichen in Übereinstimmung mit Literaturangaben die prinzipielle Gleichwertigkeit der offenen und der laparoskopischen radikalen RPVE in Bezug auf Morbidität, frühe onkologische und funktionelle Parameter bei allerdings noch signifikant längerer OP-Zeit in der Laparoskopiegruppe im Bereich der Lernkurve.
Between January 2002 and April 2003 the data of 56 patients of the urological clinic of the university of Halle who have undergone a radical prostatovesiculektomy because of a restricted prostate carcinoma have been prospectively recorded in an unrandomized way. 28 patients each have been operated by means of a retropubically open radical RPVE or rather a laparoscopic extraperitoneal RPVE. There were not any significant differences with regard to the age (65.3 years vs. 62.4 years) and the BMI (27.6vs. 25.8). The average operating time was with 220 minutes for the laparoscopic RPVE a significantly shorter one than with 144 minutes for the open RPVE (p There were not any statistically significant differences regarding such parameters as intraoperative loss of blood (p 0.422), postoperative PSA (p 0.197), length of stay of a bladder catheter (p 0.837) and hospitalisation period (p 0.262). The total amount of morbidity was 21.4& for the group of the open RPVE and 17.9% for the group of the laparoscopic RPVE at a total mortality rate of 0%. There were not any statistically significant differences either in the postoperative classification of carcinoma stages or in the rates of RO and R1 resections in both of the patients’ groups. The results of this available comparative study underline in accordance to bibliographical references the fundamental equal value of open and laparoscopic radical RPVE with regard to morbidity, early oncological and functional parameters but with a still significantly longer operating time in the laparoscopic group within the learning bend.
