Abstract: Das Basaliom ist mit einer jährlichen Rate von ca. 100 Neuerkrankungen pro 100.000 Einwohner der am häufigsten auftretende epitheliale Tumor der Haut. Aufgrund seiner pathobiologischen Merkmale nimmt das Basaliom eine "Mittelstellung" zwischen benignen und malignen Hauttumoren ein. Hinsichtlich der Aggressivität im Wachstumsverhalten ist bei vielen Tumoren ein Zusammenhang sowohl mit Veränderungen im Ploidiegrad als auch in der Mutation des p53-Tumorsuppressorgens bekannt. Ziel der Untersuchungen war es deshalb, das Verhalten des Ploidiestatus und der p53-Expression bei diesen semimalignen Tumoren zu bestimmen und mit dem histologischen Basaliomtyp zu vergleichen. Dazu wurden bei insgesamt 76 Basaliomen (35 solide Basaliome, 37 multilokuläre superfizielle Basaliome, 3 Ulcera rodentia, 1 Ulcus terebrans) bildzytophotometrisch das DNS-Verteilungsmuster und immunhistochemisch der p53-Nachweis durchgeführt. In 38% der Fälle konnte eine positive Immunreaktion nachgewiesen werden, während bei nur 5 Tumoren (7%) ein aneuploider DNS-Gehalt gefunden wurde. Ein direkter Zusammenhang zwischen dem immunhistochemischen Nachweis von mutiertem p53 und dem Ploidiestatus der untersuchten Basaliome, unabhängig von ihrem histologischen Typ, ließ sich nicht nachweisen. Die Ergebnisse zeigen, daß das p53-Protein eine wichtige Rolle in der Pathogenese des Basalioms spielt. Weitere Untersuchungen zur Aufklärung einer möglichen Beziehung zwischen positiver p53-Immunreaktion und p53-Genmutationen sind notwendig. So ist beispielsweise der Nachweis von p53-Mutationen in Basaliomen mittels PCR zu erwägen.
Basal cell carcinomas are the most frequent skin tumors with an incidence of 100 cases/100000 per year. Pathobiologically they show characteristics of benign as well as malign tumors. In various tumors a correlation between growth behaviour and either ploidy or mutations of the p53 tumor suppressor gene has been established. In the current study we, therefore, examined ploidy and p53 expression in basal cell carcinomas in comparison to the histological type of the tumor. For this purpose, the ploidy was determined by image cytometry as was p53 expression by immunohistochemistry in 76 basal cell carcinomas (35 solid tumors, 37 multilocular superficial tumors, 3 ulcera rodentia, 1 ulcus terebrans). 38% of the tumors stained positive for p53 and 7% of the tumors were aneuploid. We could not show a correlation between the mutated p53 gene product and ploidy independent of the histological type of the basal cell carcinomas examined. These results suggest an important role for p53 in the pathogenesis of basal cell carcinomas. Further studies are needed to determine the correlation between positive immunostaining for p53 and mutations of the p53 gene. It might be worth examining mutations in the p53 gene by PCR.
