dc.description.abstractZielstellung: In der Frauenklinik Suhl sind bis 1999 über 500 Mammareduktionsplastiken durchgeführt worden. Seit 1994 wenden wir eine spezielle Mammareduktionsplastik als eigene Modifikation der Strömbeck-Methode an. Das Ziel der Untersuchungen ist es, den Vorteil des kraniomedialen Pedikels gegenüber der weit verbreiteten McKissock-Technik darzustellen. Material und Methode: In den Jahren 1997 und 1998 sind insgesamt 151 Mamma-reduktionsplastiken durchgeführt und ausgewertet worden. Alle Patientinnen wurden retrospektiv untersucht bezüglich Resektatgewicht, Blutverlust, Operationsdauer, Komplikationen, ästhetischer Ergebnisse und der Patientenzufriedenheit (Fragebogen). Ergebnisse: In den zwei Jahren wurden 128 Operationen mit kraniomedialem Pedikel und 23 Operationen nach der McKissock-Methode durchgeführt. Nach eigenen Untersuchungen liegen die Vorteile der Mammareduktionsplastik mit kraniomedialem Pedikel gegenüber der McKissock-Methode insbesondere in dem geringen Blutverlust (durchschnittl. Hb- Verlust prä-/postop. : 0,8 mmol/l vs. 1,4 mmol/l), der kürzeren Operationszeit (88 Min. vs. 125 Min.), der Patientenzufriedenheit (84% sehr und gut vs. 59%), und der Erhaltung der Sensibilität des Areola-Mamillenkomplexes (81% vs. 53%). Es wird ein eigens entwickeltes Verfahren zur Vermeidung einer Areola- Mamillenretraktion in Form der retroareolären Stütznaht vorgestellt. Bei Anwendung dieser Naht trat postoperativ kein invertierter Areola-Mamillen-Komplex auf. Zusammenfassung: Die Mammareduktionsplastik mit kraniomedialem Pedikel ist eine gut erprobte, sichere, einfach praktikable und schnelle Operationsmethode für fast alle Resektatvolumina mit geringem Blutverlust und guten funktionellen und ästhetischen Ergebnissen. Durch exakte Planung und Selektionierung der Patienten lassen sich überwiegend gute Ergebnisse erzielen.-
dc.description.abstractObjectives: In our department OB/ GYN in Suhl we performed between until 1999 more than 500 reduction mammaplasties. Since 1994 we used a special technique as an own modification of the original Stroembeck-method. The purpose of this study was to compare our method of reduction mammaplasty with the international widespread method of McKissock. Study Methods: Between 1997 and 1998 we performed 151 mammaplastic procedures. All cases where checked retrospectively for resection volume, blood loss, duration of the operation, surgical complications, aesthetic deviations of the different techniques and for satisfaction of the patients by questionnaire. Results: During the two years we performed 128 surgical procedures with the technique of superomedial pedicle and 23 with the McKissock method. In summary, the described surgical technique of superomedial pedicle as our modification of the Strömbeck method is more improved concerning blood loss (0,8 vs. 1,4 mmol/l), duration of the operation (88min. vs. 125min.), satisfaction of the patient (84% very good and good vs. 59%), and maintained sensibility of the areola-nipple-complex (82% vs. 53%). A new technique for prevention of inverted nipple is described. Conclusions: The superomedial pedicle technique is a safe, suitable, simple, speedy and booldless technique for nearly all cases of reduction mammaplasty. By a correct and well-proved indication our method of superomedial pedicle has a very low complication rate.eng
