Abstract: Die Neck dissection ist ein standardisiertes Verfahren zur Entfernung maligner Metastasen im Halsbereich. Die Operation kann Schulterfunktionsstörungen zur Folge haben, die die Lebensqualität der Patienten erheblich einschränken können. Ziel der Studie war es, eine Methode zur Erfassung der Trapeziusfunktion zu entwickeln und die Auswirkungen von Schulterfunktionsstörungen auf die Lebenssituation des Patienten einzuschätzen. Wir führten eine simultane Schulterhebekraft-Messung und Oberflächenelektromyographie durch. Während der isometrischen 90° Arm-Abduktion ermittelten wir mittels einer bipolaren Ableitung des oberen und unteren Trapeziusanteils frequenz- und amplitudenabhängige Merkmale bei rampenförmig ansteigender Schulterhebekraft. Mittels des Constant-Murley-Scores beurteilten wir die Lebensqualität der Patienten. Die retrospektive Studie umfasste 71 Patienten. Nach radikaler Neck dissection und bei einer Trapeziusparese bestand ein signifikant geringerer Anstieg (p<0,05) der Root Mean Square, der mittleren Amplitude pro Turn und des Frequenzverhältnisses sowie niedrigere Werte des Constant-Murley-Scores. Die Trapeziusfunktion kann mittels der simultanen Schulterhebekraft-Messung und Oberflächenelektromyographie quantitativ dargestellt werden. Der Constant-Murley-Score ist geeignet, die Auswirkungen von Schulterfunktionsstörungen auf die Lebensqualität der Patienten zu bestimmen. Anhand dieser Methode können Schulterfunktionsstörungen beurteilt und eine adäquate Therapie eingeleitet werden.
Neck dissection is a standardised procedure to remove malign metastasis from the neck. This surgery can lead into disfunctionalities of the shoulder, that might impair daily life activities of the patients considerably. The survey had the objective to develop an approach to measure the trapezius function and to evaluate the consequences of shoulder disorders on the daily life of the patients. We conducted a simultaneous measurement of shoulder leverage and surface electromyography. During the isometrical 90° arm abduction we used a bipolar diversion of the upper and lower trapezius section to measure characteristics, depending on frequency and amplitude while an increase of the shoulder leverage energy took place (force graph with shape of a ramp). We used the Constant-Murley-Score to evaluate the patients' quality of life. The retrospective survey enclosed 71 patients. At patients with a radical neck dissection and trapezius muscle palsy we measured a significant lower increase (p<0,05) of the Root Mean Square, the Mean Amplitude per Turn and the Frequency Ratio as well as smaller values of the Constant-Murley-Scores. The trapezius function can be assessed via a simultaneous force measurement and surface electromyography. The Constant-Murley-Score is suited to evaluate the consequences of shoulder disorders in daily life activities. The performed method allows an exact evaluation of shoulder disfunctions and to initiate an adequate therapy.
