Abstract: Ziel: In dieser Arbeit wurden asymptomatische Patienten und Patienten mit einer klinischen Beschwerdesymptomatik nach erfolgter Stapeschirurgie mit Hilfe des HR-CT des Felsenbeins in der frühen postoperativen Phase untersucht. Material und Methoden: Im Zeitraum von Januar 1995 bis Dezember 2000 erhielten 37 Patienten nach einer Otoskleroseoperation in der frühen postoperativen Phase ein hochauflösendendes Felsenbein-CT in koronarer und axialer Schichtung. Ergebnisse: Alle Patienten wiesen typische postoperative Veränderungen auf. In 17 der 37 CT-Befunde waren pathologische Befunde zu erkennen. Bei 11 Patienten war eine Prothesendislokation, bei 5 Patienten Lufteinschlüsse im Vestibulum und bei einer Patientin Knochenfragmente im Vestibulum nachweisbar. Sieben Patienten erhielten eine Revisionsoperation. Die erhobenen CT-Befunde wurden mit dem klinischen Verlauf und dem Operationsbericht verglichen. In 6 der 7 Fälle korrelierte der CT-Befund mit dem intraoperativen Befund. Schlussfolgerung: Eine Bildgebung mit Hilfe der HR-CT ist in der Frühphase nach erfolgter Stapeschirurgie nur in Einzelfällen eine Indikation für eine Bildgebung, da es zu einer Überlagerung von frischen postoperativen Veränderungen mit pathologischen Befunden kommt.
Purpose: This is an analysis of high-resolution computed tomography (HR-CT) of patients with and without symptoms after stapes surgery in the early postoperative phase. Methods: We examined 37 patients who underwent stapes surgery between January 1995 and December 2000. HR-CT of the temporal bone was performed with 1 mm slice thickness and table feed in axial and coronal planes. A re-operation was performed in 7 patients. CT findings were compared with clinical follow up and operative findings. Results: All patients showed postoperative opacifications of the tympanic cavity, external auditory canal and partly of the mastoid on CT-scans. In 11 cases, there was a dislocation of the prosthesis. 5 patients showed air bubbles in the vestibule. In one case, small bone fragments were detected in the vestibule. In 6 of 7 cases, CT results were confirmed by retympanotomy. Conclusion: Due to an overlapping of normal and pathological findings in the early postoperative phase, there are only limited indication for HR-CT in this period.
