Nebeneffekte der Nutzung eines Depressionsforums : Identifizierung und Quantifizierung problematischer Ratschläge zu Antidepressiva und den erkennbaren Umsetzungsabsichten mit Methoden des Text Minings und des Natural Language Processings

Abstract

Depressionsforen zählen zu den meistgenutzten Internetangeboten für psychiatrische Patienten. Ziel der explorativen Studie war es, die Dimension unerwünschter Ratschläge hinsichtlich der Einnahme von Antidepressiva in Depressionsforen verlässlicher einordnen zu können. Hierfür wurden automatisierte und manuelle Analyseverfahren kombiniert. Das Datenvolumen umfasste 34.061 Nutzer- und 547.546 Beitragsdaten. 1.056 initiale Beiträge im Unterforum Pharmakotherapie wurden automatisiert auf Antidepressiva, unerwünschte Arzneimittelwirkungen und Indikatoren für eine intendierte Änderung der Medikation gescreent und die Erstantworten auf Indikatoren für unerwünschte Ratschläge geprüft. 58 Diskussionsverläufe mit 601 Posts wurden manuell untersucht. Unerwünschte Ratschläge wurden sowohl in der Erstantwort als auch im weiteren Diskussionsverlauf identifiziert. Vielfach wurden diese durch eine empfohlene ärztliche Rücksprache relativiert und in der Hälfte der Fälle durch die Community korrigiert.

Depression forums are among the most widely used internet services for psychiatric patients. The aim of this exploratory study was to be able to more reliably classify the dimension of unwanted advice regarding the use of antidepressants in depression forums. For this purpose, automated and manual analysis processes were combined. The data volume comprised a total of 34,061 users and 547,546 posts. 1,056 initial posts in the pharmacotherapy sub-forum were automatically screened for antidepressants, adverse drug reactions and indicators of an intended change of medication, and the initial answers were automatically checked for indicators of undesirable advice. 58 threads with 601 posts were manually examined. Undesirable advice was identified both in the initial response and in the further discussion. In many cases, undesirable advice was qualified by recommending medical consultation and corrected by the community in about half of the cases.
