Abstract: Die Entstehung von chronisch obstruktiven Lungenerkrankungen (COPD) und berufsbedingten Lungenerkrankungen weisen zahlreiche Überschneidungen auf. Der Einfluss des COPD-Schweregrades und einer beruflichen Exposition gegenüber biopersistenten anorganischen Stäuben (BAS) auf kleine Fleckschatten in Röntgenthoraxbildern (RTB) nach der Staublungenklassifikation (ILO 2000) der internationalen Arbeitsorganisation (ILO) wurde bisher nicht untersucht. Ziel dieser Arbeit war es, einen möglichen Einfluss der genannten Faktoren auf die Schattendichte nach ILO 2000 zu untersuchen. Es wurden insgesamt 125 Probanden mit und 125 Probanden ohne COPD getrennt voneinander untersucht. Die Dichte, Form und Größe kleiner Fleckschatten im RTB wurden standardisiert nach ILO 2000 ausgewertet. Die berufliche Exposition gegenüber BAS wurde mit einem Interview erhoben und anschließend ein Staubsummerwert gebildet. Es bestand kein Zusammenhang zwischen COPD-Schweregrad und der Schattendichte im RTB. Die Wahrscheinlichkeit auf eine erhöhte Schattendichte im RTB wurde durch eine berufliche Exposition gegenüber BAS erhöht.
Chronic obstructive pulmonary diseases (COPD) and occupational lung disease correspond in several steps of the pathogenesis. The influence of COPD-severity and an occupational exposure to biopersistent inorganic dust (BID) on small opacities in chest X-Ray (CXR) according to the classification of pneumoconiosis (ILO 2000) of the international labor organization (ILO) were not investigated so far. The aim of this investigation was to examine the influence of these factors on the profusion of small opacities in CXR. 125 Patients with COPD and 125 Patients without COPD were examined separately. The size, shape and profusion of small opacities in CXR were standardized evaluated according to ILO 2000. The occupational exposure to BID was ascertained by an interview and subsequently a total dust value was calculated. No significant correlation between COPD-severity and profusion of small opacities was found. Occupational exposure to BID increases the likelihood of a higher profusion of small opacities in CXR.
