Abstract: Im Sputum von Patienten mit Cystischer Fibrose liegen hohe Laktatkonzentrationen vor. Es wurde untersucht, ob Laktat den aktuellen Inflammationszustand in der CF-Lunge widerspiegelt. Dazu wurde die Laktatkonzentration in 81 CF- und 8 non-CF-Sputen spektrophotometrisch bestimmt. In jeder CF-Sputumprobe wurde Laktat gefunden (3,1 ± 2,8 mmol/L). Damit waren die Werte höher als in non-CF-Proben (1,5 ± 0,9 mmol/L). Im Sputum von Patienten mit akuter respiratorischer Exazerbation verringerten sich die Werte signifikant von 5,2 ± 3,3 mmol/L vor auf 1,9 ± 2,1 mmol/L nach Antibiotikatherapie (p=0,006). Es konnte eine signifikante negative Korrelation von Laktat und dem Lungenfunktionsparameter FEV1 (r=-0,34;p=0,012) registriert werden. Laktat im CFSputum eignet sich demnach im Sinne eines Entzündungsmarkers zur frühzeitigen Diagnose einer akuten Exazerbation und zur Überwachung einer Antibiotikatherapie als Ergänzung zur weiterhin als Goldstandard geltenden FEV1.
There are high concentrations of lactate in sputum specimens of patients with cystic fibrosis. It was investigated whether lactate is able to represent the current state of inflammation in cf lung. Lactate was measured in 81 cf and 8 non cf sputa spectrophotomatically. Results were compared to lung function. In every single cf sputum lactate was detectable (3.1 ± 2.8 mmol/L). Lactate concentrations in cf sputa were higher than in non cf sputa (1.5 ± 0.9 mmol/L). In sputum specimens of patients with respiratory exacerbations lactate concentrations decreased significantly (p=0.006) from 5.2 ± 3.3 mmol/L to 1.9 ± 2.1 mmol/L after antibiotic therapy. Significant negative correlation was detected between lactate concentration and lung function parameter FEV1 (r=-0.34;p=0.012). Successful antibiotic therapy is reflected by decreased lactate concentrations. Lactate in cf sputum may be used to recognize respiratory exacerbations precociously and monitor responses to antibiotic therapy as an adjunct to FEV1.
