Abstract: Ziele: Mit EMG-Monitoring wurde in Vorarbeiten das Signal A-Train und dieTrainzeit(TZ) als Prädiktor der postoperativen Fazialisparese(FP) identifiziert. Die Methode sollte optimiert werden. Methodik: Ein Computerprogramm wurde entwickelt, das gleich aussehende A-Trains in A-Traingruppen (ATGs) sortiert. Die Anzahl der ATGs wurde retrospektiv als Prädiktor der postoperativen FP an 28 Patienten erprobt, die an einem Akustikusneurinom operiert wurden. Ergebnisse:Die Anzahl der ATGs steht in hoch signifikantem Zusammenhang mit dem Grad der postoperativen FPbei Patienten ohne präoperative FP.Für Patienten mit vorbestehender FPwar keine signifikante Aussage möglich. Statistisch scheint die TZ keine Mehrinformation gegenüber der Anzahl der ATGs zu liefern. Diskussion: Die Anzahl der ATGs ist ein wertvolles Maß für die Prognose der postoperativen FP. Ihre Aussagekraft scheint der TZ überlegen zu sein. Eine intraoperative Anwendung sollte erprobt werden.
Object: Using electromyographic monitoring the signal-type A-Train and traintime(TT) have previously been identified as indicators for postoperative facial palsy(FP). Target to this study was an improvement of predictive values. Methods: A computer algorithm was developed to subsume A-Trains in A-Train-Groups (ATGs) applying geometric parameters. The number of ATGs is retrospectively tested as a quantitative predictor for postoperative FP on 28 patients undergoing surgery for vestibular schwannoma. Results: The number of ATGs is associated with the grade of postoperative FP in a highly significant fashion in patients without preoperative FP. For patients with preoperative FP no correlation could be shown. Statistically TT does not favour any information beyond the information in the number of ATGs. Conclusion: The number of ATGs is a valuable measure for prognosis of postoperative FP. Its conclusiveness seems to be superior to TT. An intraoperative application should follow.
