Isolierte Frakturen der Sinus frontalis Vorderwand und des Orbitadaches : eine retrospektive Untersuchung unter spezieller Betrachtung der Osteosynthesematerialien und Komplikationen

Abstract

In der Universitätsklinik und Poliklinik für Mund-, Kiefer- und Plastische Gesichtschirurgie der Martin-Luther-Universität Halle-Wittenberg wurden im Zeitraum von 2011-2017 18 Patienten mit isolierten Frakturen der Sinus frontalis Vorderwand und optional des Orbitadaches operativ behandelt. Ziel der retrospektiven Auswertung war eine genaue Betrachtung der verwendeten Osteosynthesematerialien und aufgetretenen Komplikationen. Osteosynthetisiert wurde überwiegend mit Mikroplatten (n=10), gefolgt von Titangittern (n=2), bioresorbierbaren Kunststoffen (n=2), Gewebeklebern (n=2), einem autologen Knochentransplantat und einem PSI. In 11 Fällen traten postoperativ keine Komplikationen auf. Die verwendeten Osteosyntheseverfahren konnten nahezu komplikationsfrei durchgeführt werden und stellten ein suffizientes Mittel zur Rekonstruktion dar. Aus dem eigenen Patientengut und der Fachliteratur konnten darüber hinaus Osteosynthesekonzepte für verschiedene Frakturtypen abgeleitet werden.

18 patients with isolited anterior table fractures of the frontalis sinus and the orbital roof were treated at the department of oral and maxillofacial surgery at the Martin-Luther-Universität Halle-Wittenberg between the years 2011 and 2017. The purpose of this study was a retrospective analysis, with special focus on used osteosynthesis systems and occured complications. For osteosynthesis we mainley used mikroplates (n=10), followed by titanium meshes (n=2), biodegradable materials (n=2), tissue adhesives (n=2), one autologous bone graft and one PSI. In 11 cases no postoperative complications occurred. The used osteosynthesis systems were nearly free of complications and a sufficient way of reconstruction. From our own patients and literature we concluded on osteosynthesis concepts for different types of these fractures.
