Title: Untersuchung humoraler und zellulärer Abstoßungsreaktionen bei allogener Gefäßtransplantation

Abstract: 67 Patienten mit der Diagnose „Gefäßbypassentzündung“ wurden mit allogenen arteriellen Gefäßtransplanten behandelt. Humorale Abstoßungsreaktionen im Empfängerorganismus wurden postoperativ durch Bestimmung Donor-spezifischer anti-HLA-Antikörper in den Blutseren untersucht. Neben der klinischen Beobachtung wurden die Spenderarterien mikroskopisch auf eine zelluläre Abstoßungspathologie untersucht. Bei präoperativ nicht berücksichtigten HLA-Merkmalen entwickelten 96% der Patienten nach der Transplantation anti-HLA-Antikörper, wobei Donor-spezifische Antikörper bei 89% der Patienten nachgewiesen wurde. In der Nachbeobachtungszeit stellten postoperative Thrombosierungen mit einer Rate von 18% die Hauptkomplikation des Verfahrens dar. Zwar zeigten sich histologisch keine akuten Abstoßungsreaktionen mit Organversagen, jedoch wurde in allen histologischen Untersuchungen eine chronische Abstoßung im Sinne einer Kollagenisierung aller Gewebeschichten beobachtet, welche Thrombosierungen begünstigt. Somit scheint auch bei der allogenen Gefäßtransplantation eine HLA-kompatible Gefäßauswahl sinnvoll, da somit sowohl humorale als auch zelluläre Abstoßungsvorgänge verhindert werden könnten.
67 patients were grafted with allogenic arterial vessels in order to treat infected synthetic vascular bypasses. To show transplant-specific immunization patients and donors were HLA-typed. In addition to clinical follow up allografts were histologically analyzed to observe immunological induced postoperative cellular reactions. Due to not considering HLA-histocompatibility 96% of the patients developed a humoral anti-HLA immune response with 89% of them emerging virtually definable Donor-specific antibodies. Regarding the clinical outcome 18% of the patients exhibited thromboses as the most frequent complication. Although not leading to acute graft dysfunctions comparable to those of solid organs, arbitrarily selected vessels lead to a high degree of allo-immunization with consequent chronic degeneration processes like fibrosis and collagenization and its resulting thrombosis. Thus, the overall practicability of the unmatched allocation of arterial homografts is clearly limited and allogenic vessels should be transplanted as HLA-compatible as possible to prevent immune responses and its resulting organ failure.
