Abstract: Ziel der vorliegenden Arbeit war es, zu untersuchen, ob die mitochondrialen Veränderungen in den Muskelproben von 21 ALS-Patienten im Vergleich zu 21 Gesunden und 23 Patienten mit anderen neurogenen Erkrankungen (NK) nachzuweisen und, falls vorhanden, als ALS-spezifisch zu werten sind. Histologische Veränderungen der oxidativen Enzymaktivität in beiden erkrankten Gruppen in Form von Cores und Targets sowie mtDNA/Kern-DNA-Quotienten für jede Muskelprobe sollten quantifiziert werden. Die Aktivität der Citratsynthase (CS) sollte eine Aussage zur Mitochondrienmenge der Muskelproben ermöglichen. Targets (jeweils pro 500 Muskelfasern) waren selten (je 1 bei 2 von 21 ALS-Patienten, 1 bei 1 von 23 NK). Dagegen waren Cores häufiger anzutreffen (1-7 Cores bei 7 von 20 ALS-Patienten, 59 Cores bei 1 ALS- Patienten mit stärkeren Atrophien; 1-8 Cores bei 8 von 23 NK). Der Unterschied zwischen den mtDNA/Kern-DNA-Quotienten der einzelnen Gruppen (5,07±2,08 für ALS-Patienten, 4,95±1,93 für NK, 5,38±1,75 für Gesunde) war statistisch nicht signifikant. Die CS-Werte (U/g NCP) waren im Vergleich zu Gesunden (100 %) bei den ALS-Patienten auf 117,5 %, bei NK auf 140,5 % statistisch signifikant erhöht. CS-Werte (U/g Probe) waren nur für NK signifikant erhöht, dies war jedoch durch den signifikant niedrigeren Proteingehalt der ALS-Muskelproben im Vergleich zu Gesunden bedingt. Somit ist eine Verminderung der oxidativen Enzymaktivität bei den ALS-Patienten als unspezifisch und durch chronische Denervation bedingt anzusehen. Der Mitochondriengehalt im Muskel ist insgesamt eher erhöht. Die Gesamtmenge der mtDNA sowohl bei ALS-Patienten als auch bei anderen neurogenen Erkrankungen bleibt durch eine unspezifische kompensatorische Erhöhung der Mitochondrienzahl bei erniedrigtem mtDNA-Gehalt pro Mitochondrium in beiden erkrankten Gruppen erhalten.
The aim of the present study was to investigate, whether mitochondrial changes are detectable and, if detectable, ALS specific in muscle biopsies of 21 ALS patients compared with 21 healthy controls (HC) and 23 patients with other neurogenic atrophies (NA). Histologic changes of the oxidative enzyme activity such as cores und targets in both diseased groups and mtDNA/nuclear DNA quotients for each muscle sample were quantified. The activity of citrate synthase (CS) allowed a statement about the amount of mitochondria per muscle sample. Targets (per 500 fibers respectively) were rare (1 in each of 2 of 21 ALS patients, 1 in 1 of 23 NA). Cores were more frequent (1-7 cores in 7 of 20 ALS patients, 59 in 1 ALS patient with severe atrophy; 1-8 cores in 8 of 23 NA). The difference between mtDNA/nuclear DNA quotients in all three groups (5,07±2,08 for ALS patients, 4,95±1,93 for NK, 5,38±1,75 for HC) was not statistically significant. The CS activity (U/g NCP) was significantly increased in ALS patients (117,5 %) and NA (140,5 %) in comparison with HC (100 %). The CS activity (U/g sample) was significantly increased only in NA. This can be explained by significantly diminished protein content of ALS muscle samples compared with HC. Thus, the reduction of oxidative enzyme activity in ALS patients should be interpreted as unspecific and caused by chronic denervation. The content on mitochondria in muscle is generally increased. The total amount of mtDNA remains in ALS patients and other NA almost constant due to unspecific compensatory increase of the number of mitochondria in spite of reduced mtDNA content per mitochondrium in both disieased groups.
