Title: Lokoregionäre Mammakarzinom-Rezidive nach brusterhaltender Therapie und Strahlentherapie

Abstract: Das lokoregionäre Rezidiv des Mammakarzinom nach brusterhaltender Operation und Strahlentherapie ist für die betroffenen Patientinnen prognostisch bedeutsam. In der vorliegenden retrospektiven Analyse wurden Prognosefaktoren für das ereignisfreie Überleben und für die lokoregionäre Tumorkontrolle untersucht. Nach brusterhaltender Therapie des Mammakarzinoms erhielten 347 Patientinnen in der Zeit von 1996 bis 2000 eine adjuvante Strahlentherapie. Die mediane Nachbeobachtungszeit betrug 53,3 Monate (ein bis 90 Monate). Das Gesamtüberleben nach fünf Jahren war 93,5%. Bei 21 (6,1%) Patientinnen trat ein lokoregionäres Rezidiv auf. Die 5-Jahres-Überlebenswahrscheinlichkeit für das krankheitsfreie Überleben betrug 88,3%. Nach fünf Jahren waren 93,8% der Frauen lokal kontrolliert. Die Erkrankten waren im Median 63 Jahre alt. Univariate Analysen nach Cox-Regressionsmodellen und nach der Kaplan-Meier-Methode zeigten im Hinblick auf das krankheitsfreie Überleben signifikante Ergebnisse für die klassischen Prognosefaktoren wie Lymphknotenstatus, Tumorgröße, Alter der Patientinnen bei Diagnosestellung und Rezeptorstatus. Eine prognostische Bedeutung ergab sich außerdem für das Tumorgrading, für den Menopausalstatus und die Art der systemischen Therapie. Das Tumorgrading und die Art der systemischen Therapie waren auch für das lokoregionäre rezidivfreie Überleben statistisch relevant. In multivariaten Analysen nach der Cox-Regression konnte hinsichtlich das lokalrezidivfreien Überlebens für das Alter bei Diagnosestellung, den Lymphknotenstatus, das Tumorstadium und für die Art der systemischen Therapie eine prognostische Bedeutung nachgewiesen werden. Für das krankheitsfreie Überleben sind nach der multivariaten Untersuchung das Alter, der Lymphknotenstatus, das Tumorstadium und auch der Rezeptorstatus Prognose relevant. Für die Art der systemischen Therapie trifft dies nicht zu. Die Zeitdauer von Diagnosestellung bis zum Bestrahlungsbeginn ist sowohl für die lokoregionäre Tumorkontrolle als auch für das ereignisfreie Überleben entscheidend. Sie wird von der Art der Systemtherapie bestimmt. Eine Verzögerung entsteht durch die adjuvante Chemotherapie vor Beginn der Strahlentherapie. In der multivariaten Cox-Regression trat dieses Merkmal hinter die tumorbiologischen Prognosefaktoren zurück.
Recurrences of breast cancer after breast conserving therapy and irradiation are of importance for these patients. In this retrospective investigatin prognostic factors for disease free survival and local tumor control were analyzed. From 1996 until 2000 347 patients received adjuvant irradiation after breast conserving therapy. The median follow-up was 53,3 months (1 to 90 months). Survaval after 5 years was 93,5%. In 21 (6,1%) patients a locoregional recurrence occured. The 5-year-suvival probability for the disease-free-survival was 88,3%. Afrer 5 years 93,8% of women were without local recurrance. The median age of patients were 63 years. The univariate analysis according to the Cox regression method and according the Kaplan-Meier plot demonstrated significant results for the classical prognostig factors such as lymph node status, tumor size, age of patients on diagnosis and receptor status. Of prognostic importance were also tumor grading, menopausal status and kind of systemic therapy. Tumor grading and kind of systemic treatment played also a role for the local recurrence-free survival. The multivariate analysis according the Cox regression method demonstrated prognostic significance for local recurrence-free survival, age on diagnosis, lymph node status, tumor stage and kind of systemic therapy. For disease free survival the multivariate analysis revealed prognostic relevance of age, lymph node status, tumor stage and receptor status as well. This is not true for systemic therapy. The duration from time of diagnosis until beginning of irradiation is important for local tumor control and also for disease-free survival. It depends on kind of systemic therapy. The beginning of irrediation is prolonged due to adjuvant chemotherapy. In the multivariate Cox regression analysis the beginning of irradiation was of less importance than prognostic factors of tumor biology.
