Abstract: In der vorliegenden Arbeit wurde die Expression von verschiedenen Zytokinen (TNF-α, IL-6, IL-8, IL-1β, VEGF) und die Korrelation zu Entzündungs- und Anämieparametern (CRP, Leukozyten, Hämoglobin) bei Patienten mit fortgeschrittenen gynäkologischen- und Kopf-Hals-Tumoren untersucht. Es wurden Serumproben zu Beginn, während und am Ende der Radio- bzw. Radiochemotherapie entnommen. Die Zytokinbestimmung erfolgte mittels kommerzieller Immuno-Assays. Insgesamt wurden 52 Patienten untersucht, 30 Patienten mit Kopf-Hals-Tumoren und 22 Patienten mit gynäkologischen Tumoren. Eine wesentliche Rolle spielte die Untersuchung von Zytokininteraktionen und möglichen Zusammenhängen zwischen der Zytokinexpressionshöhe und der Prognose. Es konnte aufgezeigt werden, dass enge Interaktionen sowohl zwischen den Zytokinen als auch zwischen Entzündungs- und Anämieparametern und proinflammatorischen Zytokinen bestehen. Eine Schlüsselrolle spielte das CRP. Patienten mit signifikant erhöhten CRP-Serumkonzentrationen wiesen häufig deutlich erhöhte Zytokinkonzentrationen auf und hatten eine schlechtere Prognose. Ebenso hatten hypoxische Verhältnisse im Wirtsorganismus häufig deutlich erhöhte Serumkonzentrationen mehrerer Zytokine zur Folge (VEGF, TNF-α, IL-6, MMP-9), d. h., die Expression verschiedener Zytokine kann möglicherweise durch Hypoxie induziert werden. Die Ergebnisse der Arbeit zeigen die Notwendigkeit einer multimodalen Tumortherapie und sprechen für einen möglicherweise sinnvollen Einsatz von Zytokininhibitoren (z.B.VEGF, MMP-Inhibitoren) auch in der primären Tumortherapie.
In the following study we analysed the expression of various cytokines (like TNF-α, IL-6, IL-8, IL-1β, VEGF) and the correlation between the cytokines and parameters of anemia and inflammation (CRP, Leukocytes, Haemoglobin) in patients with gynaecological and head and neck cancer. We collected samples of serum at the beginning, during and at the end of radio-or radiochemotherapy. For determination of the cytokine levels we used commercial immuno- assays. Overall 52 patients were examined, 30 of these patients with head and neck cancer, 22 with a gynaecological tumor. An important aim of the study was the investigation of cytokine interactions and the evaluation of the cytokine expression and the prognosis. We showed strong coherences between the cytokines and between parameters of anemia (or inflammation) and proinflammatory cytokines. A key role played in our study the CRP. Patients with significant increased serum levels of the CRP had often high concentrations of the investigated cytokines and a poor prognosis. Furthermore hypoxic conditions in the host organism correlated with increased levels of various cytokines (like VEGF, TNF-α, IL-6, MMP-9). Possibly the expression of different cytokines can be induced by hypoxia. Our results show the necessity of a multimodal tumor therapy. Possibly the use of cytokine inhibitors in the first-line-therapy of malignant tumors is efficient.
