Zusammenfassung: Hintergrund: Das NSCLC stellt ein großes gesundheitliches Problem in unserer Gesellschaft dar. Aufgrund dessen, dass zum Diagnosezeitpunkt oft schon ein fortgeschrittenes Stadium der Erkrankung vorliegt, kommt der palliativen Chemotherapie zur Lebensverlängerung und Symptomkontrolle eine bedeutende Rolle zu. Methoden: 1999 und 2000 wurde an der Martin-Luther-Universität Halle-Wittenberg eine klinisch orientierte Phase-II-Studie, in der NSCLC-Patienten im Stadium IIIB und IV mit Gemcitabin und Carboplatin behandelt wurden, durchgeführt. Die Ergebnisse wurden drei zeitlich davor erhobenen Patientengruppen aus ebenfalls nicht randomisierten Studien gegenübergestellt. 1. Paclitaxel-Monotherapie n=28; 2. Paclitaxel-Carboplatin-Therapie n=30; 3. Taxotere-Carboplatin-Therapie n=30. Ergebnisse: An der Phase-II-Studie mit Gemcitabin-Carboplatin nahmen 47,0 % Stadium IIIB und 53,0 % Stadium IV Patienten teil. Die Ansprechrate betrug 40,0 %, komplette Remission 7,0 %, partielle Remission 33,0 %, stable disease 30,0 %, Progression 30,0 %. Die 1-Jahres-Überlebensrate betrug 45,5 %, die mediane Überlebenszeit 51 Wochen. Nicht hämatologische toxische Wirkungen waren nur geringgradig, insgesamt hatte die Behandlung eine gute Verträglichkeit und Praktikabilität. Schlussfolgerung: Im Vergleich mit den oben genannten Studien ergeben sich soweit eingeschränkt möglich, ähnliche Ergebnisse in Ansprechrate, Überleben und Toxizität, dies entspricht auch den Ergebnissen aus der Literatur. Die Verifizierung und Differenzierung des anscheinend erreichten Plateaus bei der chemotherapeutischen Behandlung des NSCLC sollte durch weitere randomisierte Studien erfolgen.
Background: NSCLC is a great health problem of our society. As often at the time of diagnosis an advanced stage of NSCLC exists, palliative chemotherapy plays an important role regarding prolongation of life and symptoms control. Methods: In 1999 and 2000 a clinical orientated phase-II-study was conducted at the Martin-Luther-University Halle-Wittenberg to treat patients with stage IIIB and IV NSCLC with Gemcitabine and Carboplatin. The results were compared with three other patient groups from previous randomized studies. 1. Paclitaxel-Monotherapy n=28; 2. Paclitaxel-Carboplatin-Therapy n=30; 3. Taxotere-Carboplatin-Therapy n = 30. Results: 47.0 % Stage IIIB and 53.0 % Stage IV patients participated in the phase-II-study with Gemcitabine and Carboplatin. The overall response rate was 40.0 %, complete remission 7.0 %, partial response 33.0 %, stable disease 30.0 %, progression 30.0 %. The 1-year-survival rate amounts to 45.5 %, the median survival time 51 weeks. Non hematological toxicities were only low-grade and the treatment was well-tolerated and feasible. Conclusion: In comparison with the other above mentioned studies there arise as a conclusion similar results of response rate, survival and toxicity as limited as possible. This corresponds with the results of literature. The verification and differentiation of the apparently reached plateau of chemotherapeutic treatment of NSCLC should follow through further randomized studies.
