Zusammenfassung: Craniopharyngiome wachsen fingerförmig entlang eloquenter Hirnareale, wie dem Hypothalamus, und stellen deshalb einen schwierig zu behandelnden Tumor dar. Derzeit existieren zwei Behandlungsstrategien, eine konservative mit Teilresektion und Radiatio sowie eine progressive mit dem Ziel der Komplettresektion. Die vorliegende Arbeit beschreibt einen Behandlungspfad, der die Komplettresektion in einer Vielzahl der Patienten ermöglicht und lediglich mit einer höheren Rate neuer postoperativer endokrinologischer Defizite einhergeht, die gut behandelbar sind. Neuropsychologische oder hypothalamische Defizite stellen eine Kontraindikation zur offenen Operation dar, weshalb diese Patienten mit einer antiödematösen Therapie, Liquor-drainage und Zystenpunktion vorbehandelt werden. Bilden sich die Symptome darunter zurück, erfolgt die offene Resektion ansonsten, wie auch im Falle eines Resttumors, die strahlentherapeutische Behandlung. Als OP-Zugang wird, in Abhängigkeit der Tumorlokalisation und -größe, vorrangig der transsphenoidale, der bilateral subfrontale oder ein zweizeitiges Vorgehen, gewählt. Die, ggf. mehrmalige, intraoperative MRT ist hilfreich um das Resektionsausmaß zu kontrollieren und die Komplettresektionsrate zu erhöhen. Der Nachweis einer nukleären Akkumulation von β–Cathenin trägt zur Differentialdiagnose zystischer Sellaläsionen bei und weist auf ein adamantinöses Craniopharyngiom hin. Der Nachweis einer cDNA-Expression für den ER-1 und den hGH-Rezeptor dient darüber hinaus als Entscheidungskriterium im Hinblick auf eine endokrinologische Substitutionstherapie.
Craniopharyngiomas are difficult to treat as they exhibit invasive growth within relevant areas of the brain like the hypothalamus. For this reason, two treatment strategies exist, a conservative one consisting of partial resection of the tumor followed by radiation therapy and a more progressive one intending total removal. In this paper, a treatment path is described enabling total tumor removal in a high proportion of patients accompanied only by a high number of new postoperative endocrinological deficits which are, however, accepted as they can be easily treated. As neuropsychological and hypothalamic disturbances are contraindications for open surgery, those patients are pre-treated applying anti-oedematous therapy as well as drainage of CSF and cyst content. Only in case of resolving symptoms, open surgery takes place, otherwise radiation therapy is applied like is done in case of tumor remnants. Depending on size and localisation of the tumor, a transsphenoidal, a bilateral subfrontal or a combined approach is used for open surgery. Intraoperative MRI is useful in order to determine the extent of tumor removal and to increase the rate of complete resection. Nuclear β-Cathenin accumulation in immunohistochemistry is beneficial in differential diagnosis of cystic lesions indicating adamantinomatous craniopharyngiomas. Evidence of hGH or ER-1 receptor-cDNA expression is helpful in decision making regarding hormonal replacement therapy.
