Abstract: Ziel dieser Arbeit war die Untersuchung der apikalen Dichtigkeit und das Extrusionspotenzial von Thermafil®-Wurzelfüllungen in Abhängigkeit von unterschiedlichen Thermafil®-Stift-Größen bei standardisierter Aufbereitungsgröße. Material und Methode: In dieser Studie wurden 90 einwurzlige Zähne verwendet. Die Zahnlängen wurden mit einer Hedström-Feile der ISO 10 gemessen und die Arbeitslängen 1 mm kürzer festgelegt. Die maschinelle Aufbereitung erfolgte mit dem ProFile-System .04 bis zu einer Kanalgröße ISO 40, unter stetigem Spülen mit Natriumhypochlorid und Chlorhexidin. Alle Proben wurden auf 5 Versuchsgruppen zu je 18 Zähne aufgeteilt und mit dem Sealer AH Plus in Kombination mit ThermaFil-Obturatoren der Größen ISO 40 (Gruppe A; Kontrollgruppe), ISO 35 (Gruppe B), ISO 30 (Gruppe C), ISO 25 (Gruppe D) und ISO 20 (Gruppe E) gefüllt. Nach der Obturation wurde der Apex auf überpresstes Füllmaterial untersucht. Die Oberflächen der Zähne, ausgenommen des apikalen Bereiches, wurden mit Nagellack versiegelt. Nach einer zweiwöchigen Lagerung aller Zähne in 5%iger Methylenblaulösung wurden die Wurzeln in Längsrichtung mit einer Trennscheibe aufgeschliffen. Mit einem Stereo-Auflichtmikroskops wurden die Penetrationstiefen der Farbstofflösung entlang der Wurzelfüllung gemessen. Ergebnisse: Die Untersuchung von Überpressungen ergaben folgende Ergebnisse (überpreßter Sealer / Guttapercha +Sealer/ Guttapercha): Gruppe A: 6/4/1, Gruppe B: 5/4/0, Gruppe C: 5/3/0, Gruppe D: 3/3/0, Gruppe E: 5/0/0. Mit abnehmenden Obturatorgrößen nahm die Anzahl an Proben mit überpresstem Füllmaterial ab. Die folgenden Penetrationtiefen wurden gemessen (in ìm, mit Standardabweichung): Gruppe A: 1917 (+/- 721); Gruppe B: 1517 (+/- 584); Gruppe C: 1483 (+/- 485); Gruppe D: 3367 (+/- 795); Gruppe E: 4101 (+/- 1601). Die statistische Analyse zeigte signifikant höhere Penetrationstiefen für die Gruppen D und E und keinen signifikanten Unterschied zwischen den Gruppen A, B und C (p Diskussion: Nach der Analyse aller in vitro Ergebnisse ist zu schlussfolgern, dass die Verwendung von kleineren Obturatoren (ein bis zwei ISO-Größen) keinen negativen Einfluss auf die apikale Dichtigkeit hat.
The aim of the present study was determine the apical microleakage after root canal filling with different sizes of Thermafil obturators in an in vitro investigation. Method: Ninety extracted single rooted incisors stored in saline were used. Eighteen roots per group were endodontically prepared using the Profile-system to a maximum size of ISO 40 under constant sodium hypochloride and chlorhexidine irrigation. The root length was determined by insertion of a size 10 file into the canal until it appeared at the apex. The working length was recorded 1 mm shorter. The roots were filled with the sealant AH plus and Thermafil obturators size 40 (group A), size 35 (group B), size 30 (group C), size 25 (group D) and size 20 (group E). At first the apex of all samples were analyzed, whether sealant or obturator material was extruded. Than the teeth were coated with nail varnish, 1-2 mm of the apex cut off and stored in 5% methylene blue solution for two weeks. All teeth were splinted along their long axis. Linear dye penetration (apical to coronal) was measured using a light stereo-microscope. Statistical analysis was performed using SPSS 10.0. Results: The results of extrusion: (Extrusion material Sealer/ Guttapercha+Sealer/Guttapercha) group A: 6/4/1, group B: 5/4/0, group C: 5/3/0, group D: 3/3/0 and group E: 5/0/0. The described analyze of the extrusion results showed that the number of extrusion samples is in correlation with reduced obturator size. Following mean values of linear dye penetration were recorded (in ìm, with standard deviation): Group A: 1917 (+/- 721); group B: 1517 (+/- 584); group C: 1483 (+/- 485); group D: 3367 (+/- 795); group E: 4101 (+/- 1601). Statistical analysis revealed significant higher penetration depths in group D and E and no significant differences between group A, B and C (p Conclusion: Within the limitations of an in vitro investigation it can be concluded that using Thermafil obturators smaller than 30 might increase the risk of apical leakage.
